SPORTS SIGN-UP PROCEDURE


FALL SPORTS:

1) Report to the school nurse as soon as possible to:

a. Check on your current physical examination date and shot information.

b. If needed, schedule a physical exam with the school nurse prior to school closing this June, or with your private doctor prior to the last scheduled “Summer Walk Through Dates” and prior to your final tryout date.

c. NOTE: The school district does not do any physical examinations over the summer months. If you do not have a current exam and elect not to have an exam by the school nurse prior to the closing of school, you will need to schedule one with your private doctor. Please use the school physical exam form which can be found on line.

2) Athletes need to attend pre-season sign-up meetings with the coaching staff. Announcements will be made in the buildings or signs will be posted. You can also contact your physical education teacher or coach for information on meetings.

3) PARENTS: Complete the “Pre-Participation Evaluation Form”. DO NOT date it until 30 days prior to the start of the sport season tryouts (Modified teams – after August 4th) (JV & Varsity – after July 20th). This form is on line.

4) ATHLETES must attend the “Summer Walk Through Dates” with the school nurses (dates are listed under the “Summer Nurses’ Dates” – location for these dates this year, Summer 2007, will be at the Durgee Jr. High nurses’ office).


a. If you needed an updated physical exam, bring a copy of the exam on our school form and completed by your physician to the “Walk Through”.

b. Make sure all your shots are up-to-date.

c. Student/Athletes must attend the walk through on the date your sports are assigned. No one can pick up your “release” for you – the athlete must be present for the school nurse to process the paperwork.

5) All paperwork must be completed before the student is eligible to tryout. All paperwork needs to be completed by the 1st day of the tryouts and students must be present and ready to participate at the indicated start date and time for their particular sport.

6) Attend the “Walk Through Date” designated for your sport.

7) If you were injured or had any medical issues keeping you out of physical education or athletic participation this past year, see the school nurse to assure she has on file your medical release from your doctor to resume participation. If injured or had an operation over the summer months, you will also need a medical release from your doctor.


WINTER / SPRING SPORTS:
1) Check with the school nurse one month prior to the first day of practices to verify that your physical exam is current.

2) Parents: must complete the “Pre-Participation Evaluation Form” and sign and date it no sooner than 30 days prior to the first day of practice.

3) Attend the pre-season sports meeting for important information.

4) Be eligible for the first day of practices and have all your paperwork completed.

 

Sport Physicals
A student who participates in interscholastic athletic competition must have a current health examination, complete the “Pre-Participation Physical Evaluation” form, and do a walk through with the school nurse. The walk through must take place with the school nurse within 30 days prior to the start of any new season.

The student may schedule a medical examination with the school nurse during the school calendar year. Physical examinations will not be given by our district staff over the summer vacation (you must use your private physician).

The results of the physical shall be valid for a period of 12 months up to the last day of the month in which the physical was conducted. If the 12 month period for the physical exam expires during a sport season, participants may complete the season as long as a health history was conducted prior to the season. For example, a physical conducted on August 1 would be valid through August 31st. If a student plays beyond August (ex. football), the student may complete that sports season as long as an interval health history was conducted before the start of the season. Sports season includes tryouts.

NOTE: The handbook committee recommends that each incident of prolonged absence should be reviewed individually by the coach and the athletic director, consulting with the school physician when necessary and a reasonable amount of practice time and playing time be established, based on the athlete’s readiness for safe return to competition
