|Attention Parents: Important Health Information for 2018-19
Please note that the grades for required physicals have changed and so has the required exam form.
Beginning July 1, 2018, all NYS public school students must have a health exam when they are entering the district for the first time and entering grades K, 1, 3, 5, 7, 9, & 11. These exams should be reported on the new approved NYSED Student Health Examination Form, which you can download by clicking HERE. Have your physician fill out this form and return it to you so that you can provide it to your child’s school nurse before the beginning of the school year.
Required Meningococcal Vaccine
All students entering 7th, 8th and 9th grade must have the meningococcal vaccine. Students entering 12th grade must have had two doses of the vaccine or, if one dose was administered after your child turned 16 years old, your child only needs one dose.
The district is mandated to exclude students that do not meet this requirement. Meningococcal meningitis is a serious disease that can lead to death within hours. Survivors may be left with serious disabilities, including loss of limbs and paralysis. Please speak with your doctor regarding the vaccine. Even children who have had a shot before may need a booster. More information can be found at www.health.ny.gov/diseases/communicable/meningococcal/.
For more information on required physicals and immunizations your child may need for the 2018-2019 school year, visit the nurses’ page HERE.
