It is the responsibility of the Parent/guardian to provide the following documentation at the time of registration; not the school in which the child was formerly enrolled. Registration is completed in person at our District Administrative Office Building with the following documents presented:
Complete Registration Packet as well as provide all of the following documents:
PROOF OF AGE -- (one of the follow documents are suitable for submission):
1. Original Birth Certificate
2. Passport and/or VISA
PROOF OF RESIDENCY -- One of the following documents are suitable for submission:
I.E. contract to purchase a primary resident; lease agreement; utility invoice; driver's license with a Baldwinsville address; or a notarized affidavit from owner of private property.
IMMUNIZATIONS & HEALTH APPRAISAL -- Copy of the most recent Health Appraisal and immunization document must be dated within the last calendar year at time of registration.
DENTAL CERTIFICATE -- Complete the Dental Health form with your Certified Dentist as proof of Comprehensive Dental Examination.
PROOF OF CUSTODY (for parents separated or divorced) -- Please be prepared to provide your Custody or Divorce Document in order to establish information for our database. If a Court Order of Protection exists for your child, this must be provided to the district in order to comply with that order from the Courts.
HOME LANGUAGE QUESTIONNAIRE -- NYS requirement that all students registering needs to complete upon entry into a district. Please complete this form in its entirety. Forms are available in other languages upon request at the Registration Office.
STUDENT RECORDS:
Transcripts & Educational Records -- A copy of the students latest report card or class schedule from their prior school is required at time of registration. Be sure to have a complete district name and school name transferring from as well as phone number and contact name. We will requisition formal education and health records after registration is completed.
If your child has an (IEP) "Individualized Education Program" or "Section 504 Accommodation Plan", a copy of a plan is required in order for us to continue the support services for your child as of the first day of entrance into our schools. If these documents are not presented at the time of registration it may delay the start of your child entering our district while we retrieve the document from the former school. Your assistance with providing us a copy at registration will ensure proper placement and transition your child into school faster.
Breakfast and Lunch Program:
Applications for free or reduced meals are available at the registration office. Please be sure to request a form. If you have a direct certification letter from the NYS Office of Temporary & Disability Assistance, please present this at time of registration in place of completing the application.
