Registration for the current 2012-2013 school year for students entering Grades K through 12 for our district are taken in our Registration Office. Please be sure to print either the Kindergarten Packet with pamphlet "Kindergarten-A Beginning Chapter" or select the Grade 1 though 12 Registration packet below. Registration for students entering Kindergarten for school year 2012-2013 will be taken for student who will become 5 years of age before or on December 1, 2012. There are no exceptions.
Our registration personnel will take registration packets for students entering for school year 2013-2014 beginning on Friday, March 1, 2013 at the District Office. Please be sure to print a copy of the registration form listed below.

Registration is completed in person at our District Administrative Office Building. It is the responsibility of Parent/guardian to provide the following documentation at the time of registration (not the school in which the child was previously enrolled):
- COMPLETE REGISTRATION PACKET printed from one of the links above
- PROOF OF AGE -- (one of the follow documents are suitable for submission):
Original Birth Certificate, Passport and/or VISA
- PROOF OF RESIDENCY -- The following documents are suitable for submission:
contract to purchase a primary residence; lease agreement; utility invoice; or a notarized affidavit from owner of private property. In addition, please plan to present your driver's license with a district address or picture identification.
- IMMUNIZATIONS & HEALTH APPRAISAL -- Most recent Health Appraisal and immunization document must be dated within the last calendar year at time of registration.
DENTAL CERTIFICATE -- A Dental Health form signed by your Certified Dentist as proof of Comprehensive Dental Examination should be submitted to the Health Office for your child's school at the next scheduled visit for an examination by your dentist or orthodontist.
PROOF OF CUSTODY (for parents separated or divorced) -- Please be prepared to provide your Custody or Divorce Agreement in order to establish information for our database. If a Court Order of Protection exists for your child, this must be provided to the district in order to comply with order from a Court. If you have SOLE Custody, you must provide documentation.
STUDENT RECORDS you should bring with you to registration:
A copy of the students last Report Card and/or Student Schedule from their previous school is required at time of registration. If you are entering grades 10 through 12, you should include a copy of the STUDENT TRANSCRIPT with your documents. Be sure to have a complete district name, school name and address for the school in which you are transferring from as well as phone number and contact name. We will requisition official education and health records after registration is completed.
If your child has a Special Education (IEP) "Individualized Education Program" or "Section 504 Accommodation Plan", please present a copy of a plan is required in order for us to continue the support services for your child as of the first date of entrance into our schools. If these documents are not presented at the time of registration it may delay the start of your child entering our district while we retrieve the document from the former school. Your assistance with providing us a copy at registration will ensure proper placement and transition your child into school faster. A CSE Meeting may be required to review your students education plan and its implementation by our district before the student begins classes in our district.
Breakfast/Lunch Program:
Applications for free or reduced meals are available at the registration office. Please be sure to request a form. If you have a direct certification letter from the NYS Office of Temporary & Disability Assistance, please present this at time of registration in place of completing the application.
